Our team of highly skilled and qualified nurses offer a range of services to assist with your healthcare needs, including:

Chronic Disease Management: Working with you and your doctors to find ways to manage your condition at home

Diabetes Management: Assistance with care planning, administering insulin, wound care, and clinical monitoring

Wound Management and Stoma Care: Help to dress and manage wounds, and stoma maintenance

Continence Management: Assessment and care plans to help you maintain your lifestyle

Medication Management: Our nurses help you to manage your medication, including injections, tablets, and creams

Plan Management

We strive to help you achieve your goals by managing your National Disability Insurance Scheme (NDIS) funds, including making payments to providers, expense claims processing, providing monthly statements to participants and claiming for payment from the NDIA.

Transport and travel: such as planning their journey, accessing public transport and trouble-shooting issues
